]]>The project by its name in French was DMP which stands for dossier médical personnalisé and is nothing other than a complete electronic, digital health record for every citizen in France. Its inaugural moment was 2004 and since that moment €500 million were invested. The original objectives were several:
The first challenges that the healthcare system faced in terms of deploying the DMP program was IT-related. Most reasons given by doctors, hospital managers and other involved personnel were related to the inefficiencies of the computer program and the time-consuming tasks involved with the loading of the data. Since the creation of the program, many managers have gone by and also internet providers but none has yet met the specifications and demands of the health care professionals.
In the recent past, another obstacle is that most of the responsibility is in the shoulders of General Practitioners who are the least benefited from it. Also, they don´t receive any money for taking the extra task and uploading retroactively all the patient´s records to the system. This lack of enthusiasm and of incentive caused that only around 6,000 professionals are currently using the system and by the year 2015 only 500,000 dossiers (DMP) were created over a population of 65 million people.
The main difference between the current day and the moment that the program was implemented 15 years ago is the data governance by the users themselves. In the current scheme, the end-user of the program decides what data is included, who can see it and when and where it will be published. For example, a patient can avoid putting on mental health records on his or her dossier and when it reaches the hands of the new practitioner, the professional will never know about those records.
The challenges, benefits and consequences of DMP correct implementation are many and not to be overlooked. The entire world will be watching the turn of events for France and wishing for the best.

French Government treats the access and provision of healthcare as a national worry and is included in the agenda every year. Since the year 2001, healthcare is assured for all legal French Residents. There is a compulsory payment that is deduced from the paycheck of every working person. The 65 million people population provides the income that funds most of the healthcare system of the country. The government dedicates almost 12% of the GDP to healthcare which is one of the highest in the world.
The reach of the system is very wide and despite all the efforts being made by the national government, the access to health is very different not only for different economical stratus but also different parts of the country. Mental health access specifically is one of the most uneven of them all and it was reported that more than 70% of the psychotropic drugs sold in France come from general practitioners´ prescriptions. Big cities hold the highest amount of professionals and sometimes the distances and waiting lines in public hospitals can be long.

France´s budget for healthcare is very high, the sixth in the world and the third in Europe. But it is not only budget what the French society needs to achieve optimum health care for all citizen alike. There is a much more profound subject that has to be tackled and is very closely related to social inequality. Those coming from lower economic stratus tend to be more vulnerable and the prevention efforts done by the French Government seem to be useless or, at least, not as effective as they would like to. Health inequalities and social inequalities can´t be divorced as a subject of national concern and the French State´s effort could be directed towards extending the network and reaching those in more desperate need rather than concentrating on the big cities.
The principle of National Solidarity over which the welfare State in French is based on speaks about sharing the responsibility throughout the society as a whole and as such, it compels the government to reach out for those in need. The levels of obesity and lung cancer due to cigarettes abuse are way higher in the most vulnerable stratus of society. This is an indicator that concerns the health system.
Prevention as the first step of an integral health solution needs to be applied to those sectors of society where it is needed the most. The effort that the French Government is doing should be directed towards assisting those in more desperate need rather than flooding the big cities with state-of-the-art facilities for those whose wallet allows them to assist. Inequality starts with income and affects healthcare and not the other way around.

France takes healthcare very seriously and every year almost 12% of the GDP goes to health-related issues. There´s a part of that budget that is dedicated to the care of the older, disabled and prevention and the other part is destined to the execution of the healthcare practices themselves in the hands of practitioners, physicians and such. This is the first big promise about the Statutory Healthcare Insurance: every legal resident of France will have equal access to the best healthcare.
While it is the big promise, reality shows differently and the healthcare system in France has a very distinct reach to different economical stratus despite government efforts. This is evident in many ways, for example the access to practitioners is radically different in the rural areas and the big cities. Not so much because of the budget dedicated to it by the government but basically because the system is not branched out efficiently enough. Rather than the amount of money is the money distribution that makes a difference.
Another weak point of the system is the mental health sector in which the specialists and general practitioners have uneven percentages of prescribing medication. There is currently a flat fee for the general practitioners that is different (lower) than the one being charged by specialists of a certain matter. This results in people going to mental health consultations to general practitioners because of a money issue. Furthermore, this increases the gap going against the very foundations of the system, because the ones who have access to the specialists are the ones who can afford it and live in an area where there is one available. No psychological treatment is funded by the State leaving those who can afford it in the hands of a psychiatrist and taking medication to solve any kind of trauma or mental illness.
While the healthcare system in France has a long way to go, there are efforts being made from the government to close the gap and have an integral coverage for all citizens. The importance of the SHI for French people is huge and it can be thought as a leap into a brighter future. The process of implementation will involve closing the gaps currently existing in the programs and when that happens, the population will be fully covered.
